Go into the toilets on the 2nd level. See all the sinks and turn on the taps. Shoot the guy in the toilet and you should hear a gunshot to confirm that you have done it correctly. .

To get an all weapons upgrade just go to secret unlocks and type in Q LAB…… be sure to put the space between Q and LAB otherwise it wont work.
