Artifact Temple
Collect twelve Artifacts. When given the choice to warp to a different level, select Artifact Temple. Simply entering will unlock it.

Expert Mode
Beat Mission Mode.

Beat the six-ball mini-game in Artifact Temple and select to go to Impact Crater, then defeat Metroid Prime.
