Extra lives
Get the 1UP near the end of Stage 4-2. Then go back up the ladder and out of that area. Return, and the 1UP is still there to get again. Repeat for extra lives.

Let Title screen fade to black, press Start, Start, then hold Up+Left+Select+A+B and press Start.
