Wrinkly Kong, DK64. Having not read the instruction manual (I practically never do), I had no idea what was behind the doors in the lobby. So I walked up to one, thinking there was probably a mini-game behind it (you know how much Rare loves mini-games), and instead a ghost pops out and shrieks at me! Re-deads, LoZ: OoT. I sure wasn't expecting to find brain-sucking zombies in the Hyrule Castle Market after the whole 7 year leap.

Happy Mask salesman, LoZ: MM. First off, I never trusted this guy. Second of all, he's a PSYCHO!

Dante's ghost, LoZ: OoT. The way he laughs was rather unsettling. Plus, his face is still deformed even as a ghost. And I got a slight suspiscion that he wanted to kill me.

[EDIT] I got one more. The SA-X, Metroid Fusion. Although she shows up very rarely, I knew that I'd eventually have to fight her, and I could never tell when. That's what's so freaky about her. She could just pop up at any time and annahilate me. That kind of stuff makes a person paranoid.

Lord Doskias in Septerra Core is pretty scary, but that's mainly because he takes Gemma's blade out and kills Gunnar in one hit.

Pikachu's pretty freaky too.

Anyway,SA-X always freaked me out.Just the way it slowly walked around was scary enough.*tap-tap-tap* Like Rayman FREAK said,the thought it could pop outta nowhere and obliterate me at any time was scary.And remember near the beginning?Those eyes...Brr...

Oh,and that one time,it was walking around,so I rolled into a ball and hid in this space under the floor.It walked to the right,jumped down,and was going to walk through the door.I thought it was safe to get on the platform above it,and THE FREAKIN' FLOOR COLLAPSED!I start frantically running for my life,only to get obliterated with a couple ice beams...That was a classic video game moment...

And I just remembered something else that does creep me out...
I dunno if anyone will even know what I am talking about, but...
In the game .hack//INFECTION, on some of the random levels, there's this big thing that floats/spins around in the sky every once in a while... Kinda looks like some freaky HUGE mummy type thing. o_O;; I just think it's kinda creepy looking for some reason...and weird. I mean...why would that be floating around in the sky? o-o; Odd.
