Just politely decline & say you're not looking right now.

Somebody's grandmother once gave me her granddaughter's phone number in my checkout line. That was awkward.

Be relatively truthful; "Thank you, but I'm really not actively seeking a relationship right now". Optional extra, "because I'm focusing on work/school/other".

Well, I'd say upfront that you aren't interested, but still meet the person. It doesn't hurt to make new acquaintances and friends...

Eh, I usually go "uhhhh what's this for?" or tell them I'm interested in the opposite sex or that I'm asexual, or that I'm someone they don't want their contact to bring home... or if I'm in a REALLY nice mood, than it's the "Oh I'm sorry, but I don't need help in the relationship department, thanks tho!"
