The most useful ones:
The Fire Flower has always been good. There's nothing quite like that satisfying *pop* you hear when you blast an enemy away with a fireball.
Starmans/Rainbow Stars are good while they last, of course.
The Raccoon Leaf is one of the best flight items in the series. The Cape Feather is okay, and lets you fly farther, but with much less control over your flight. The Carrot is useful for timing your jumps, even if you can't really fly in it. The Red Star is cool, but rather pointless since it only appears in the hub world. The Propeller Mushroom is one of the more useful ones.
The Hammer Suit great because you can kill almost anything with the hammers. They're a little hard to aim, though.
Kuribo's Shoe is awesome. It's a shame it only appears in one level.
The Metal Cap is the best powerup in SM64, since it works a lot like a Starman.
The Mega Mushroom is a blast.
Overall, I think either the Hammer Suit or Kuribo's Shoe is my favorite.

The least useful ones:
The Frog Suit is great for underwater levels, but a real pain in the arse on land.
The Wing Cap is one of the most difficult flight items to control. It does what it's supposed to, yet you don't really fly in the Wing Cap as much as you just fall with style.
The Spring Mushroom is just a major pain to use, and probably my least favorite powerup.

The next powerup I'd give Mario would be... a Bullet Bill Suit. The suit allows you to rocket yourself horizontally, plowing through enemies or shooting safely over wide pits.

Favorite: Hammer Suit/Metal Cap (DAT MUSIC)
Least Favorite: Spring Mushroom (dat control)

Most Useful: Propeller Mushroom. The Cloud 'shroom's pretty useful too.
Least Useful: Frog Suit/Red Star

I don't have any answers for the first few questions, but I would give him a diet plan as a next power up.

From the List:

Fav: Frog Suit, Tanooki Suit, Hammer Suit, Fire Flower, Red Star, and Wing Cap.
Hate: Spring Mushroom.
Useful: P-Wing, all Stars.
Useless: P-Balloon.

But... as I see from the list, none of the Mario Party Items are on that list. Yes, I count them as Powerups. Considering that, my real list would add Fav and Useful a Reverse Mushroom (Mario Party 3). It has MULTIPLE applications... in the game where it came from.

Frog Suit because I hate water levels. Also it was kinda bad on land, I think. (this is the part where someone reveals it is in fact the best power-up and using it correctly lets you breeze through the game and unlock a secret ending where Bowser surrenders immediately before turning a new leaf)

Red Star. It's cool but...you can use it only in Galaxy's hub. Lame.

NEW POWER-UP: Giga Mushroom: it turns Mario so big he crushes the world, letting him move onto the next one. Using one locks you into a 'bad' ending, in which Mario becomes addicted to its powers and decides to crush everything. Luigi has to team up with Bowser to put him down. It'll be a very sad scene. (Luigi will have a sad face and a violin will play a sad song as the camera does a close up of Mario's bloody cap that has a sad face drawn on it)

or, uh...Electric Flower! It shocks enemies! Plus, when you do a 'spin-attack', you'll release a lot of electricity and go higher somehow! And, while we're at it, it'll power generators! Powering generators is what the Mario series is all about!

New Power: Spear Mario. In a 2D adventure, Mario would get a spear he could throw. Also, it would stick in walls and serve as a platform.

Least favorite is probably the P-Wing. I just never used it.
