Where is it? Whatever became of it? Why isn't it ever mentioned again (that I know of)?

Termina never existed. Majora's Mask was just a bizarre dream Link had not long after he acquired the Goron's Ruby in Ocarina of Time, while he was drugged and being made a "Sworn Brother" by Darunia.

You can dress it up however you want, but it's actually just an obvious excuse for Nintendo to reuse existing models, textures, and so on from Ocarina.
